A completed Personal History Statement (PHS) must be received as part of this process, and one has been provided for your convenience. Once you complete the Word file, please send us the electronic copy. There is no need to sign it before you send it in. This will be completed at your background interview with your investigator. Please complete on an IBM-compatible computer (no Apples/Macs) as it will not format properly.
